Our THN Physician Liaison Team is the primary channel of communication between THN and provider offices.
We provide:
- Practice and physician onboarding, education and support through the Elite Primary Care Model
- Physician incentive education
- Quality performance packages
- Reducing out-of-network utilization through referral management software
- Physician and staff town halls, and meetings to keep you up to date
- Participate in patient-centered campaigns within the community like health fairs and annual wellness visit campaigns to keep our patients healthy
