Advance Care Planning - Practice Resources

Research shows patients want to remain in control of their care and discuss options with the provider who knows them best – their family physician. Using advance care planning, their health care decisions reflect their values, preferences and conversations with loved ones.

THN can support your efforts to weigh options with patients and help them make a plan. Advance care planning should be part of every patient’s health care journey regardless of their age or current health conditions.

We provide your practice with the tools and resources to support your conversations with patients and their families. We offer training, events and other resources. Also, we share current data on how our ACO is performing in terms of advance care planning conversations, documentation and billing.

Resources for Patient Conversations

To support your advance care planning conversations with patients, we offer how-to guides and scripts.

For patients who may benefit from palliative care, we encourage you to consult clinician guidelines and consider training and guidance on other resources.

For patients requiring hospice care, there are many local hospice and palliative care resources.

In North Carolina, end of life wishes must be notarized.

Training for Advance Care Planning

For all Vynca users, we offer the following training:

  • Viewing advance care planning documents
  • Uploading documents
  • Creating electronic medical orders for scope of treatment (MOST) forms
  • Creating a Goals of Care document
  • Frequently asked questions

For non-Epic Vynca users, we also offer training on uploading documents. In addition, we offer training on billing and facilitating conversations with patients.

Providers with a Cone Health email address may access the Center to Advance Palliative Care (CAPC) facilitating conversations and advance care planning billing guide.
