Patient Attribution Management

One of the ways THN provides optimal patient care is through attribution management. Typically, patients select a primary care provider (PCP) during open enrollment or payers assign one through claims filled by a provider. We use this process to identify the beneficiaries associated with THN and to define the patient population for which THN is accountable.

At times, patients need to be reattributed for various reasons. Our quality team manages reattribution requests and works with payers and practices to ensure each patient is attributed to their PCP.
