Our History
2010: A small group of visionary physician leaders met to collaborate on the future of health care. Their goal was to provide cost efficient, high quality care to the Piedmont Triad community. As a first step, they formed a clinically integrated network (CIN) to bridge the fee for service and emerging bundled payment system, which led to the creation of Triad HealthCare Network (THN) as an Accountable Care Organization (ACO).
2011: Our population health strategy was launched. Participating providers received additional rewards for managing the patient population in a more efficient manner and achieving specific quality benchmarks.
July 2012: Through the Centers for Medicare and Medicaid Services (CMS), THN was selected to participate in the Medicare Shared Savings Program as an ACO.
2013: Our evolution toward risk included forging strategic partnerships, outsourcing and covering Humana HMO patients’ health care costs, including services and prescriptions, in exchange for full premiums.
2015: HealthTeam Advantage, a Medicare Advantage Plan and wholly-owned subsidiary of Cone Health, was launched.
January 1, 2016: THN was selected to participate in the Next Generation ACO, which:
- Builds on experience from the Pioneer ACO Model and the Medicare Shared Savings Program.
- Supports ACOs experienced in coordinating care for patients and whose provider groups are ready to assume higher levels of financial risk and reward.
- Provides quality, cost-efficient and patient-focused care.
2017: Commercial risk-based reimbursement plans were added.
2017: THN received the National Association of ACO’s (NAACOS) Innovation Award for its home health ReDS™ system strategy, which helps reduce readmission among heart failure patients.
2019: THN served 198,000 patients in the Piedmont Triad area of North Carolina; this was twice as many patients served in 2018.
